A 49-year-old man from Mount Carmel, Pa., has died following a single-vehicle accident that happened on Monday on Interstate 80 west in Lackawannock Township, according to report.
Puchalski was traveling westbound in a Toyota Tundra near the 8.8 mile marker when the accident happened at about 4:37 p.m.
According to police, Puchalski’s truck traveled off its lane before striking a culvert, then a ditch. He pronounced dead at UPMC Horizon Shenango in Farrell where he was taken by McGonigle Ambulance Service.
